  • Medication Therapy Management Program

    The Medication Therapy Management Program (MTMP) is a feature of PHP (HMO SNP). MTMP can help people who take multiple prescription drugs, have chronic illnesses, and/or expect to spend a significant amount of money on prescription drugs each year.

    MTMP helps to identify:

    • Possible medication errors
    • Drugs that may not be necessary because you are taking another drug to treat the same medical condition
    • Drugs that may not be safe or appropriate because of your age or gender
    • Certain combinations of drugs that could harm you if taken at the same time
    • Prescriptions written for drugs that have ingredients to which you are allergic
    • Possible errors in the amount (dosage) of a drug you are taking.

    If we see a possible problem in your use of medications, we will work with your provider to correct the problem. MTMP provides current information on proven medical practices to you and your provider to determine the most effective treatment. The program also helps you understand your condition and medications, so you can take an active role in managing your health.

    All members of the plan are automatically enrolled in the MTMP. There is no extra cost for the program and you have the option to not participate in the program at any time.

    Your registered nurse care manager (RNCM) will contact you to complete an initial health assessment within 60 days from the date of your enrollment in the plan. During the assessment, your RNCM will conduct a comprehensive medication review, which is a key part of the MTMP. The initial health assessment usually takes up to 60 minutes to complete. Your RNCM will conduct periodic health assessments and medication reviews for you during the year. The frequency and duration of these periodic assessments depend on your health and care plan. You will also receive a comprehensive medication review during your annual health assessment. This assessment usually takes under 60 minutes.

    Your RNCM will provide you with MTMP materials like your Medication Action Plan and Personal Medication List after your health assessments and medication reviews. If you lost your copy of these materials or did not receive them, please contact your RNCM. You can also download a blank copy of the Personal Medication List that you may use to track your medications.

    If you choose to not participate in the MTMP, please MTMP opt out form, complete and return it to us at the address shown on the form. You can also call Member Services and tell us that you do not want to participate.
