Positive Healthcare Disease Management Program

Since 1999, the Positive Healthcare Disease Management Program continues to serve Florida’s HIV-positive Medicaid population with quality health care coordination and education.  The program, operated by AIDS Healthcare Foundation’s Managed Care Division, provides disease management services to Florida MediPass recipients who are HIV-positive and Project AIDS Care Waiver recipients who are not enrolled in a Medicare or Medicaid health maintenance organization (HMO), provider service network (PSN), minority provider network (MPN) or any other non-MediPass Medicaid managed care organization.

Florida’s Agency for Health Care Administration (AHCA), which oversees Florida Medicaid, contracts with AIDS Healthcare Foundation to provide disease management services to eligible Medicaid recipients throughout Florida under the Positive Healthcare Disease Management Program brand. The program currently cares for more than 6,000 enrollees statewide.  There is no cost to Medicaid recipients to enroll in the program.

The Positive Healthcare Disease Management’s purpose is to keep its members as healthy as possible so that they may lead productive and fulfilling lives. The program provides its members education and tools to equip them to:

  • Manage their health condition and make informed lifestyle decisions to improve their health
  • Understand and adhere to their treatment plans and drug regimens
  • Become active participants in their health care and work in partnership with their providers
  • Navigate the health care system so that they receive the health care they need
