Starting in 2026, there will be some changes to PHP (HMO SNP)’s costs and benefits about which you should be aware.  You should have received the 2026 Annual Notice of Changes, notice where to download the Evidence of Coverage and Comprehensive Formulary (effective January 1, 2026; last updated October 8, 2025), and Evidence of Coverage Rider for People Who Get Extra Help Paying for Prescription Drugs (only if you get “Extra Help”) at your home or mailing address.  If you did not receive these publications and would like paper copies, please contact Member Services.

The Annual Notice of Changes explains the changes to PHP for 2026.  It also explains what to do if you want to change plans and when you need to take action if you do want to change plans.
