If You Move

Our plan is available to Medicaid beneficiaries in Broward, Miami-Dade and Monroe Counties only.  Our service area is Broward, Miami-Dade and Monroe Counties.  If you move within Broward, Miami-Dade and Monroe Counties, you can stay in the plan.   If you move outside of our service area, you will need to pick a new managed care plan in your new county.

Please call Member Services to give us your new address as soon as possible.

You must also give your new address to the Department of Children and Families (DCF) or the Social Security Administration (SSA).    (By clicking on the above links, you will be taken to the DCF or SSA website, neither of which are operated by PHC Florida.)  You can call the DCF at (866) 762-2237 or call the SSA at (800) 772-1213.  The DCF or SSA must have your current address.

If you move out of PHC Florida’s service area, you must call the Enrollment Broker Helpline to pick a new managed care plan.   Call (877) 711-3662.  TTY users call (866) 467-4970.  The Enrollment Broker Helpline is open Monday through Thursday, 8:00 a.m. to 8:00 p.m. and Friday 8:00 a.m. to 7:00 p.m.  Have your Medicaid ID number available when you call.
