PHP (HMO SNP) members may select a gym membership at one of the following gym chains listed below AND up to $550 worth of over-the-counter (OTC) pharmacy (non-prescription drug) items every plan year for no cost and no copayment.

To make or change your gym selection, contact Member Services; or download the Fitness Benefit Election Form, complete it and send it to Member Services. If you are continuing your enrollment in PHP to a new plan year, you may change your gym selection once a plan year between January 1 and January 15. (A plan year is January 1 through December 31.)

• AHF Fitness Center – One location at 4905 Hollywood Blvd., Los Angeles, CA 90027.  For more information about this location, go to the AHF Fitness website.
• 24 Hour Fitness – The 24 Hour Fitness membership may be used at any 24 Hour Fitness facility. To find a gym closest to you, go to the 24 Hour Fitness website. (By clicking on this link, you will be taken to a website operated by 24 Hour Fitness and not PHP.)
• LA Fitness/Esporta Fitness – The LA Fitness/Esporta Fitness membership may be used at any LA Fitness or Esporta Fitness facility except LA Fitness Signature Clubs.  The membership can only be used at locations in the state in which you reside.
