PHP (HMO SNP) has developed a library of HIV basics and HIV-related health condition and drug information fact sheets. Feel free to download any of the fact sheets below if you have a question or concern about a specific health condition or drug. Remember, you can always contact your Case Manager or primary care provider (PCP) if you have health questions or concerns. Your PCP’s phone number is on your member ID card. If you have a health question after hours, weekends or holidays and you cannot reach your RN Care Manager or PCP, please call the After Hours Nurse Advice Line.
