Community Health Assessments and Community Health Improvement Plans
Public health departments that are accredited by the Public Health Accreditation Board must conduct Community Health Assessments (CHAs) on a regular basis and use those assessments in the development of their Community Health Improvement Plans (CHIPs).
PHC California, as a Medi-Cal managed care plan, is an important stakeholder in these public health activities. Accordingly, we conduct outreach to accredited public health departments in Los Angeles County with the goal of meaningful participation in future assessments and improvement planning. To help guide our participation, we solicit input from our Public Policy and Community Advisory Committee (PPCAC).
Los Angeles County Department of Public Health
The Community Health Assessment (CHA) Unit is a part of the Office of Health Assessment and Epidemiology within the Los Angeles County Department of Public Health. The CHA Unit oversees the Los Angeles County Health Survey, which was most recently conducted in 2023. The survey collects data on a vast range of health topics, including demographics, social determinants of health, health behaviors, health care access and utilization, health outcomes, health opinions, and more. Results of the 2023 survey are available on the website of the Office of Health Assessment and Epidemiology. While PHC California was not a participant in the 2023 survey, the plan will pursue opportunities to contribute to the next one, which will most likely be conducted in 2026, 2027 or 2028.
The Los Angeles County Department of Public Health’s Community Health Equity Improvement Plan for 2024-2029 combines the Community Health Improvement Plan and the Center for Health Equity’s Action Plan. PHC California is currently in talks with county representatives about how the plan can support the county’s goals related to sexually transmitted infections.
City of Long Beach Department of Health and Human Services
Long Beach Department of Health and Human Services does not have a published Community Health Assessment or Community Health Improvement Plan on its website. PHC California has contacted the department for more information.
Pasadena Public Health Department
The Pasadena Public Health Department conducted a Community Health Assessment in 2025 and has published an Executive Summary of its 2023-2028 Community Health Improvement Plan. While PHC California was not invited to participate in the 2025 CHA, the Health Plan is pursuing future opportunities to collaborate.
