To find a PHC California network provider or facility (like a hospital or urgent care center) you can:
- Search online using our Network Provider and Facility Search application

- Download and view our entire PHC California Provider Directory (effective September 1, 2024).
If you want a copy of any of the above directories, please ask Member Services to send you a copy.
