Long Term Care (LTC) Providers
LTC providers are essential partners in meeting Medi-Cal beneficiaries’ long-term care needs. PHC California’s provider network includes:
- Freestanding and hospital-based skilled nursing facilities (SNFs)
- Freestanding and hospital-based subacute care facilities
- Intermediate care facilities for the developmentally disabled (ICF-DDs)
LTC Virtual Office Hours
Network providers are invited to join PHC California’s LTC Virtual Office Hours. Health plan representatives will be available to answer questions about covered services, claims payment, dispute resolution, care management, transitions of care, and Medi-Cal incentive programs such as SNF WQIP.
